SHOULDER SURGERIES

SHOULDER PAIN IS AN ENORMOUSLY COMMON AND THERE ARE MANY CAUSES ASSOCIATED WITH THE PROBLEM. ARTHRITIS IS ONE OF THE COMMONESTCAUSE IN ELDERLY PEOPLE. IT IS A CONDTION WHICH AFFECT CARTILAGE OF JOINTS. SHOUDER ARTHRITIS CAUSESCARTILAGE OF JOINT TO WEAR OUT THIN & EXPOSE THE OPEN BONE .AS CARTLLAGELIINING WEAR AWAY THE DEFENSIVE LINING BETWEEN THE BONES WILL BE LOST&THIS IS MAIN CAUSE OF SHOULDER PAIN .MUSCULAR SHOULDER ARTHRITIS IS VERY PAINFUL & CAN RESULT IN THE CONSTRAINT OF NORMAL ACTIVITIES. The pain may be tolerated with use of some medication for some time,But there may come a time when surgery is required.

Shoulder is third common joint after knee shoulder which require replacement for treating pain due to arthritis.

Total or Partial shoulder Replacement with prosthetic joint reduces pain and restore independence . surgeon install artificial ball and joint into shoulder.. The ball is removed from the top of the humerus and replaced with a metal implant.. The metal implant is shaped like a half moon &attached to a stem inserted down the center of arm bone.. The socket portion of joint is replaced with plastic socket , cemented into scapula.

People who experience pain in shoulder with developed arthritis or structural injury of shoulder are often accompanied by intense pain & inability to raise arm above shoulder level , soft tissue damage to rotator cuff.. ( group of muscles stabilizing the shoulder ) can render person unable to perform daily activities like bathing or dressing.

When damage to Rotator cuff is severe , then Surgeon may advise REVERSE TOTAL SHOULDER REPLACEMENT SURGERY ( RTSA)

In RTSA , socket prosthesis replaces rounded end of humerus and metal ball is installed into gleroidfussa ( shoulder socket) ..This allows delloid muscle to become elevator in shoulder instead damaged and torn rotator cuff muscle.

ARTHROSCOPIC BANKART REPAIR

Aim of procedure is to repair torn labrum & ligments of shoulder to prevent likely hood of dislocation

Surgery

  • Usually have regional nesthesia supplement with genral anesthesia
  • Usually overnight stay or day care admission
  • Arthroscopy -3 small cut wounds made to allow passage of arthroscope (camera)& instruments into shoulder.
  • Bioabsorbable anchors are inserted into socket after small holes are drlled,sutures attached to anchors are passed through torn labrum & repaired back to glenoid bone.
  • Postoperative Arm is immobilized in slingforapprox. 3 weeks.
  • Strengthening program is started subsequently in 6-8 weeks.

ARTHROSCOPIC ROTATOR CUFF REPAIR

Rotator cuff is group of muscles which control movements of shoulder & contribute to stability. They are :Supraspinatus,Infraspinatus,Subscapularis,Tered minor .

  • Depending on severity
  • Physiotherapy
  • Surgery – not improving with conservative treatment
  • Can be repaired open or arthroscopic / key hole surgery

FROZEN SHOULDER RELEASE

Clinical condition wherein the patient has restricted active and passive glenohumeral (Shoulder) motion for which no other cause can be identified ( X ray to be normal). Exact cause is not known. Multiple causes has been identified :

  • – Improve your posture of sitting and Standing
  • – Apply Gentle Heat
  • – Antiinflammtory or pain killer as per prescribed
